Assuming you don't want to transpose to more mainline stuff with e.g.
3. Nf3 cxd4 4. Nxd4 d6/e6/Nc6 5. Nc3, why not 3. e5 Ne4?! 4. Qe2 looks
strong at a first glance, or perhaps 3. ... Ng8!? 4. Nf3 cxd4 5. Nxd4
Qa5+ 6. Nc3 Qxe5+ 7. Be2 seems reasonable, or 3. Bc4 Nxe4 4. Bxf7 Kxf7
5. Qh5 (a bit like the line in the Cochrane)

There are around 20 or so games at URL:http://www.chesslive.de and
URL:http://www.chesslab.com that start that way - can't quite get
the copy and pasting working at the minute on this box

My rough search reveals it's rare, but it has scored well for Black :-)
1. e4 c5 2. d4 Nf6
ECO : B21 B22
47 games selected, 46 results known
Overall : 35.9%
Next Moves :
d5 (0/0/1) 0.0%
Nf3 (0/0/2) 0.0%
dc5 (1/0/4) 20.0%
Nc3 (1/0/3) 25.0%
e5 (10/3/16) 39.7%
Bd3 (1/0/1) 50.0%
c3 (1/0/0) 100.0%
Bc4 (1/0/0) 100.0%

and after 3. e5 :
1. e4 c5 2. d4 Nf6 3. e5
ECO : B21 B22
30 games selected, 29 results known
Overall : 39.7%
Next Moves :
Ng8 (1/0/0) 100.0%
Nd5 (2/2/3) 42.9%
Ne4 (7/1/13) 35.7%

so 3...Ne4 has scored well, though after
1. e4 c5 2. d4 Nf6 3. e5 Ne4? 4. f3
Black loses a piece:
4... Qa5 5. c3
5... Nxc3 6. Nxc3 cxd4 7. Qxd4 Nc6 8. Qd5
5... cd4 6. fxe4 (or 6. Qxd4 Nc5 7. b4 Qc7 8. Qxc5) 6... Nc6 7. Nf3
4... e6 5. fxe4 Qh4+ 6. Kd2 Qxe4 7. Nf3 cxd4 8. Qe2

After 3...Nd5 then 4. c4 (4.c3 is the Smith-Morra main-line)
ought to be good for White, but the game's not over yet:
1. e4 c5 2. d4 Nf6 3. e5 Nd5 4. c4
4... Nc7?! 5. dxc5 e6 6. Nf3 Bxc5 7. Nc3 d5 (or 7...Nc6)
4... Nb4!? 5. dc5 (or 5.d5 d6 6.a3 N4a6 7.Nf3)
5... e6 6. Nf3 Bxc5 7. Nc3 O-O 8. Be2
5... d6!? 6. ed6 Bf5!? 7. Na3 Qa5 8. Bd2 Qxc5 is unclear :-)

I am guessing that your smiley is that you are joking. But I am pretty
sure most Sicilian players are not playing for the draw. And the vast
majority of the decisive games are for white. 23/6.

It is interesting that the two replies that seemed most obvious dc5 and
e5 were the 2 most common responses.
